What is Prostate Cancer?

Prostate cancer is one of the most common cancers affecting men. It develops in the prostate gland, a small gland located below the bladder that plays an important role in the male reproductive system.

Many prostate cancers grow slowly and may not cause symptoms in the early stages. Early detection allows for more treatment options and improved outcomes.

Common Symptoms

Early prostate cancer may not cause any symptoms. As the disease progresses, patients may experience:

  • Difficulty passing urine
  • Weak urine stream
  • Frequent urination, especially at night
  • Blood in the urine
  • Blood in the semen
  • Erectile dysfunction
  • Pelvic discomfort
  • Bone pain in advanced disease

It is important to note that these symptoms can also occur in non-cancerous conditions such as an enlarged prostate (BPH).

Risk Factors

Factors that may increase the risk of prostate cancer include:

  • Increasing age
  • Family history of prostate cancer
  • Genetic factors
  • Obesity
  • Certain inherited gene mutations

PSA Screening and Early Detection

What is PSA?

PSA (Prostate-Specific Antigen) is a protein produced by the prostate gland and measured through a simple blood test. Elevated PSA levels may be associated with prostate cancer, but can also occur due to benign prostate enlargement (BPH), infection, inflammation, or other non-cancerous conditions.

Why is PSA Screening Important?

Prostate cancer often develops without causing symptoms in its early stages. PSA testing can help detect prostate cancer before symptoms appear, allowing treatment to be started at a stage when outcomes are generally more favorable.

Who Should Consider PSA Screening?

PSA screening may be considered for:

  • Men aged 50 years and above
  • Men with a family history of prostate cancer
  • Men experiencing urinary symptoms
  • Individuals at higher risk based on their medical history

The decision to undergo PSA screening should be individualized after discussing the potential benefits and limitations with a urologist.

Does a High PSA Mean Cancer?

Not necessarily. An elevated PSA level does not automatically indicate prostate cancer.

PSA levels may increase due to:

  • Benign Prostatic Hyperplasia (BPH)
  • Urinary tract infections
  • Prostatitis (prostate inflammation)
  • Recent urinary procedures
  • Prostate cancer

Further evaluation may include repeat PSA testing, MRI of the prostate, and prostate biopsy when appropriate.

Diagnosis

Early diagnosis is crucial for successful treatment.

Evaluation may include:

  • Detailed medical history and examination
  • Digital Rectal Examination (DRE)
  • PSA (Prostate-Specific Antigen) blood test
  • Multiparametric MRI (mpMRI) of the prostate
  • MRI-guided or TRUS-guided prostate biopsy
  • PSMA PET-CT scan in selected patients

Staging and Risk Assessment

Once diagnosed, prostate cancer is classified based on:

  • PSA levels
  • Gleason Grade Group
  • MRI findings
  • Biopsy results
  • Extent of disease spread

This information helps determine the most appropriate treatment plan.

Treatment Options

Treatment depends on the stage of cancer, overall health, age, and patient preferences.

Active Surveillance

Selected patients with low-risk prostate cancer may be safely monitored with regular PSA testing, imaging, and follow-up examinations.

Radical Prostatectomy

Radical prostatectomy involves complete removal of the prostate gland and is commonly recommended for localized prostate cancer.

This procedure may be performed using:

  • Open surgery
  • Laparoscopic surgery
  • Robotic-assisted surgery

Radiation Therapy

Radiotherapy may be used as a primary treatment for localized prostate cancer or in combination with other treatments.

Hormonal Therapy (Androgen Deprivation Therapy)

Hormonal therapy reduces testosterone levels, which helps control the growth of prostate cancer cells.

Chemotherapy

Chemotherapy may be recommended for advanced or metastatic prostate cancer.

Advanced Systemic Treatments

Selected patients may benefit from newer therapies, including:

  • Novel hormonal agents
  • Targeted therapies
  • Immunotherapy in selected cases
